We are actively hiring Registered Nurses (RNs) in the Philippines to join a major US-based healthcare initiative focused on care coordination and chronic disease management. As a Care Coordinator, you will play a critical role in improving the health outcomes of patients in the United States β helping them avoid unnecessary hospitalizations through proactive support, education, and follow-up care.
This is a full-time night shift role (aligned with US Eastern Time), ideal for nurses who are highly organized, fluent in English, and committed to making a difference in patientsβ lives.
π Key Responsibilities
- Serve as the primary care coordinator for assigned US-based patients
- Conduct telephonic outreach and/or in-person assessments (based on program guidelines)
- Collaborate with physicians, specialists, and team members to create individualized care plans
- Educate patients and caregivers on disease management, medications, and lifestyle adherence
- Track key health indicators and intervene early to prevent ER visits or hospital readmissions
- Coordinate services across multiple healthcare providers, facilities, and support systems
- Maintain timely, accurate records in EHRs or program-specific platforms
- Report clinical outcomes and escalate high-risk cases as needed
- Participate in care team meetings and contribute to ongoing planning and improvements
β Must-Have Qualifications
- Active RN license (Philippines)
- Experience in chronic care, patient education, or case management
- Excellent English communication skills (both verbal and written)
- Strong clinical judgment and attention to detail
- Comfortable working independently and using telehealth systems
- Willing and able to work night shift hours (US Eastern Time)
- Tech-savvy and familiar with electronic documentation

- Background in US healthcare programs, utilization review, or transitional care
- Familiarity with electronic health record (EHR) systems
- Prior experience in a remote or telephonic nursing environment
- Training in motivational interviewing, behavioral health, or patient advocacy
